It's not uncommon to need to adjust several times before determining the most effective one for your child's ADHD symptoms. It is crucial that parents are involved in the  titration process  and be on the lookout for their child's behavior so they can let the doctors know whether the medication is helping or causing adverse reactions.

If you decide to go back to NHS treatment, you will receive a private prescription for your medication. You will have to pay the pharmacy. This is not included in the consultation cost. You'll be required to attend six monthly appointments to monitor your medication. This includes pulse and blood pressure readings. Annual exams are also required.

Follow-up appointments

After the assessment is completed you will be able to make appointment with the psychiatrist as necessary. Usually the appointment will take place through a video call using zoom, although in certain cases it might be possible to schedule an audio only consultation (again at an extra cost).

We will then discuss the options for treatment and then decide on a bespoke, individualised ADHD medication titration. This will include an assessment of your medication dosage and a discussion around any adverse effects you may be experiencing. These titration sessions cost PS150 for face-to-face sessions, and PS60 by video or phone consultation. Medication costs are additional and are paid directly to the pharmacy.

The psychiatrist will ask questions regarding your family history, the effects of your ADHD symptoms on your daily life and any mental health issues that you may be experiencing. ADHD can be a genetic condition, and it's vital to examine your family history in order to determine if you're at a greater chance of developing this condition.

During this examination, it is essential that you are truthful and honest with your doctor in the sense that they will be using the information you provide to determine if you meet the diagnostic criteria for ADHD. This is a crucial step, as a wrong diagnosis could lead to inappropriate medication prescription.

If you are unsure of which one you prefer, we suggest that you speak to your GP before booking your private examination as some providers have different rules regarding whether they require the submission of a GP referral letter or not. Check if your GP accepts the Shared Care Agreement, which means you will be referred to them when you leave the clinic for medication prescription.

Medication

For many people who seek an individual diagnosis of ADHD, medication is a key element of their treatment. The process of getting an appointment with an NHS psychiatrist can be a hassle and lengthy. NHS waiting lists are notoriously long, and GPs may be reluctant to refer patients to specialist services.

Inquiring your GP to refer through Right to Choose is one way to avoid this (see psychiatry UK and ADHD 360). This means that you can choose a provider that offers an assessment and titration. This can cut down on wait time by a significant amount and is more convenient if you're working and have a hectic schedule.

After you've been referred through this route, your chosen clinician will assess you and decide whether or not you require to be adjusted on your medication. You will be asked to fill out forms that are different for each provider, but the most important ones are self-report questionnaires and medical histories from childhood and adulthood. These forms will assist your doctor determine any issues that could mean you shouldn't take ADHD medications if they're prescribed.

If you are on a titration medication, this will usually consist of stimulants and non-stimulant drugs. They will be assessed and monitored to determine the right combination for you. The titration can be long and may require a number of additional appointments before you are stable.

The frequency of your follow-up appointments is contingent on the type and quantity of medication you're taking. They can be monthly or quarterly. The appointment will involve your doctor checking that you are taking the correct medication, logging any side effects you've experienced, and making any adjustments needed to ensure that you're on a safe dosage. You'll also have to attend a regular health check, which could include blood pressure readings, pulse and weight as well as an annual review.

Some patients may be content with their medication, and do not have to discuss their titration in more detail. If, however, they do need further discussions about their titration and treatment plans, it's possible to be passed back to the private clinic that made their initial assessment. In this case the titration is included in the consultation cost. The medication will be administered via a private prescription. This can cost more than an NHS prescription because the pharmacy will charge for the medication.
